May 10, 2006 Firm's Recall Hotline: (866) 879-7877 Juicy Couture Children's Jewelry Recalled for Lead Poisoning Hazard WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. Name of Product: Juicy Couture Children's Jewelry Units: About 2,800 Manufacturer: Liz Claiborne Inc., of North Bergen, N.J. Hazard: The recalled jewelry contains high levels of lead. Lead is toxic if ingested by young children and can cause adverse health effects. Incidents/Injuries: None. Description and Models: The recalled bracelets and necklaces are gold or silver with charms. The charms include a yellow painted metal shirt with "Viva La Juicy" printed on the front, a cheerleader's megaphone with the word "Juicy," a green heart with "Juicy Couture," a black dog, the letter "J," and a purple flower. The jewelry is sold in a pink box with "Juicy Couture" and "Made with Love G&P" printed on the top. Style numbers YJRU0722, YJRU0723, YJR0724 and YJRU0538 are included in this recall. The style number is printed on a white sticker on the bottom of the pink jewelry box. Sold at: Department stores nationwide from September 2005 through April Manufactured In: China Remedy: Consumers should immediately take the recalled jewelry away from children and return it to the place where purchased for a full refund. FOR IMMEDIATE RELEASE Adventure Playsets Recalls Swing Sets - Detaching Frames Pose Fall Hazard WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. Name of Product: Adventure Playsets Wooden Swing Sets Units: About 26,000 Distributor: Adventure Playsets, dba Backyard Ventures Inc., of Amarillo, Texas. Hazard: Defective bolts could cause the swing set frame to detach from the fort structure, posing a fall hazard to the user. Incidents/Injuries: Backyard Ventures has received 64 reports of the bolt heads twisting off during installation. No injuries have been reported. Description: The recalled swing set models include: The Durango, Tacoma, Oddsey, Bellvue, Monarch, Grand Teton and Outlook II. The swing sets are made of wood, and feature various types of slides, swings and a fabric canopy over part of the structure. The Durango, Tacoma, and Odyssey models are pictured below, additional models can be viewed at www.adventureplaysets.com Sold at: Wal-Mart, Menards, and Toys "R" Us stores nationwide and Wal-Mart.com from February 2006 through March 2006 for about $1,000. FOR IMMEDIATE RELEASE Rainbow Play Systems Recalls Swing Seats for Fall Hazard WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. Name of Product: Sling Swing Seats Units: About 18,400 Importer: Rainbow Play Systems Inc., of Brookings, S.D. Manufacturer's Agent: Super Tech S. Corp., of Arcadia, Calif. Hazard: These swing seats can unexpectedly break in half, causing the users to fall to the ground. Incidents/Injuries: Rainbow has received 84 reports of broken swing seats, including one report of an injury. A 2-year-old girl suffered a broken wrist when she fell to the ground after the sling swing unexpectedly broke in half. Description: The recall involves sling swing seats sold as an accessory with Carnival, Sunshine, and Rainbow Series residential play structures manufactured by Rainbow Play Systems, Inc. The seats are about 25 inches in length and were sold in red, yellow, blue and green with 64-inch yellow or green dipped chains. The seats have pointed ends with three black dots and black grommets. Printed on the seat bottom is: Warning Play structures with the sling swings contain a metal plate on the main beam with the writing, "Rainbow Play Systems Inc. 1-800-RAINBOW." Sold at: Rainbow distributors and dealers nationwide from July 2004 through February 2005 as part of Carnival, Sunshine and Rainbow Series play structures that cost between $1,500 and $2,000, or about $50 to $60 for the swing seat when purchased as a stand alone accessory. Manufactured In: China Remedy: Consumers should immediately stop using these sling swings and contact Rainbow Play Systems, Inc. or return them to their place of purchase for a free replacement swing. Rainbow has contacted consumers directly who purchased the recalled sling swing seats. Consumers who have already had their swing seats replaced by Rainbow are not affected by this recall. May 25, 2006 WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. Name of Product: "Baby 2 Pack" Pacifiers Units: About 3,600 packages of two pacifiers. Importer/Distributor: KI, also known as Kole Imports, of Carson, Calif. Hazard: The pacifier's nipple can easily detach from the guard, posing a serious choking hazard to young children. Incidents/Injuries: None reported. Description: The recalled pacifiers are rubber nipples attached to a white or light blue plastic guard. The cardboard packaging has a picture of a baby, some toy blocks, a toy train engine and a diaper pin with a bear on it. The pacifiers are sold in packs of two. Writing on the front of the packaging includes, "Baby," "2 pack Pacifiers" and "BI194." The back of the packaging has an American flag and the writing, "AN AMERICAN COMPANY." Sold at: Discount dollar stores nationwide from August 2005 through March 2006 for about $1.
