The Little Tikes Co. Recalls Animal-Shaped Flashlights Containing Lead Paint Sold at Target
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of Product: Glowin' Dino and Glowin' Doggy Animal Flashlights
Units: About 20,800
Manufacturer: The Little Tikes Co., of Hudson, Ohio
Hazard: The light green paint on the dinosaur-shaped flashlight and the brown paint on the dog-shaped flashlight could contain excess levels of lead. Lead is toxic if ingested by young children and can cause adverse health effects.
Incidents/Injuries: None reported.
Description: The Glowin' Dino Flashlight and the Glowin' Doggy Flashlight are about 9-inches long and make an electronic sound when switched on. The green dinosaur roars, and the white and brown dog barks. Only flashlights with the following date codes are in included in the recall: LC5H161, LC5H291, LC5I031, LC5I091, LC5I131, LC5J061, LC5J231, and LC5J311. The date codes are found on the bottom of the flashlights. Units that do not contain these codes or other animal flashlight characters made by The Little Tikes Co. are not included in this recall. Also, units that have an "R" at the end of the date code are not included in the recall.
Sold at: Only at Target stores nationwide from November 2005 through December 2005 for about $10.
Manufactured in: China
Remedy: Consumers should immediately take the recalled flashlights away from young children and contact Little Tikes for more information.
Consumers will get a free replacement flashlight or refund.

Hazard: The recalled jewelry could break, releasing small beads that pose an aspiration hazard to young children.

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of Product: "U.S. Polo Association" Youth Hooded Sweatshirts with Drawstring
Units: About 30,000
Manufacturer: Jordache Ltd., of New York, N.Y.
Hazard: The garments have a drawstring through the hood, posing a strangulation hazard to children. In February 1996, CPSC issued guidelines to help prevent children from strangling or getting entangled on the neck and waist by drawstrings in upper garments, such as jackets and sweatshirts.
Incidents/Injuries: None reported.
Description: The recalled youth hooded fleece garments have drawstrings.
They were sold in a variety of colors and have "U.S. Polo Assn." written on them. A sewn-in tag also reads, "U.S. Polo Assn". The style number for these garments is 12-5169.
Sold at: JC Penney stores nationwide from September 2005 through December 2005 for about $40.

Remedy: Consumers should remove or cut the drawstrings to eliminate the hazard, or return the garment to the store where purchased for a refund.

Name of Product: Walgreens High Potency Iron Supplement
Units: About 64,000
Manufacturer: Inverness Medical Nutritionals Group, of Freehold, N.J.
Hazard: The iron supplements are not in child-resistant packaging as required by the Poison Prevention Packaging Act. Ingesting multiple iron supplements at once can cause serious injury or death to young children.
Incidents/Injuries: No injuries or incidents have been reported.
Description: The recalled supplements were sold in a brown plastic bottle. "Walgreens Finest High Potency Iron Supplement" is printed on the yellow and green label on the bottle.
Sold at: Walgreens retail stores nationwide from April 2005 through October 2005 for about $5.

Remedy: Consumers should keep this product out of reach of children and return it to Walgreens for a free replacement.

Reebok Recalls Bracelet Linked to Child's Lead Poisoning Death
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of Product: Reebok Heart-Shaped Charm Bracelets
Units: About 300,000
Importer: Reebok International Ltd., of Canton, Mass.
Hazard: The recalled jewelry contains high levels of lead, posing a risk of lead poisoning and adverse health effects to young children.
Incidents/Injuries: Reebok has received a report of a death caused by lead poisoning of a 4-year-old child from Minneapolis, Minn. The child reportedly swallowed a piece from one of these bracelets.
Description: The recall involves an 8-inch long metal bracelet with a heart-shaped charm. The name "Reebok" is engraved on the one side of the charm. The charm bracelet was provided as a free gift with the purchase of various styles of children's footwear.
Sold at: Major shoe stores nationwide from May 2004 through March 2006.
The manufacturer's retail price of the shoes ranged between $33 and $50.

Remedy: Consumers should immediately take the charm bracelets away from children and dispose of the entire bracelet.

Name of Product: Dollar Tree Mood Necklace and Ring, Glow-in-the Dark Necklace and Ring, and UV Necklace and Ring

Hazard: The recalled jewelry contains high levels of lead, posing a serious risk of lead poisoning and adverse health effects to young children.

Description: The rings are silver in color, adjustable, and have one of a variety of designs with a toy "gem" in the center. The necklaces have a black string with silver colored clasps and a silver charm with a "gem" in the center. The following are printed on the charms' packaging:
"Mood Necklace," Mood Ring," "Glow in the Dark Necklace," "Glow in the Dark Ring," "UV Necklace" or "UV Ring." The "UV" jewelry packaging reads, "The Sun's Energy Will Change The Color." On the reverse of the packaging is "SKU#815485" and the name "Mannix."
Sold at: Dollar Tree, Dollar Bill$, Dollar Express, Greenbacks, Only $1 and Super Dollar Tree stores nationwide from September 2003 through February 2006 for $1.

Child's Death Prompts Replacement Program of Magnetic Building Sets
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of Product: All Magnetix Magnetic Building Sets
Units: About 3.8 million
Importer: Rose Art Industries Inc., of Livingston, N.J.
Hazard: Tiny magnets inside the plastic building pieces and rods can fall out. Magnets found by young children can be swallowed or aspirated.
If more than one magnet is swallowed, the magnets can attract to each other and cause intestinal perforation or blockage, which can be fatal.
This product is unsuitable for young children.
Incidents/Injuries: CPSC is aware of 34 incidents involving small magnets, including one death and four serious injuries. A 20-month-old boy died after he swallowed magnets that twisted his small intestine and created a blockage. Three children ages ranging from 3 to 8 had intestinal perforations that required surgery and hospitalization in intensive care. A 5-year-old child aspirated two magnets that were surgically removed from his lung.
Description: All Magnetix magnetic building sets including the X-treme Combo, Micro, and Extreme sets. The sets contain 20 to 200 plastic building pieces and 20 to 100 1/2-inch diameter steel balls. The building pieces are red, yellow, blue and green, and are shaped in 1 1/2-inch squares, 1-inch triangles and cylinder rods. Some plastic building pieces have "Magnetix" imprinted on them.
Sold at: Wal-Mart, Target, Toys R Us, Fred Meyer, Design Science Toys Ltd., A.C. Moore, and other toy and arts and crafts stores nationwide.
The Magnetix magnetic building sets were sold from September 2003 through March 2006 for between $20 and $60, depending on the size of the set. The replacement program does not include sets at retail.

Remedy: Consumers should stop using the magnetic sets and return the sets to Rose Art for a free replacement product suitable for young children under the age of 6. Consumers should be sure to keep all small magnet parts out of the hands of children who mouth objects, especially children under the age of three.
