Hours: 8:00am-5:00pm, 24 hour on call.

Serving seriously ill persons and their families during the process of dying and grieving. Patient care is provided in homes, and area hospitals and nursing homes by the an inter-disciplinary team including, RN, Social Worker, Medial Director, Home Health Aide, Chaplain and Volunteers. Eligibility Requirements: Children and adults dealing with the death of someone in their lives. LOVED ONE NEED NOT BE HOSPICE PATIENT

The Community Hospice Grief Centers provide professional support, education, information and resources to the grieving in our com­munity. Services include, specialized Support Groups for Grieving Children, Teens and Adults, Educational Presentations, Personal and Family Grief Counseling, Referrals, Resources and Community Crisis Intervention.

We provide comprehensive professional sup­port to families whose infant dies suddenly and unexpectedly of any cause. The center offers educational programs to professionals and the community at large on SIDS, Risk Reduction, Shaken Baby Syndrome prevention, infant mortality, and bereavement. We are a state­wide program with 4 satellite offices in Albany, Buffalo, Liverpool and NYC. Our main office is in Stony Brook and can be reached at 1-800-336-7437.
